Patients and Evolutionary History
Medical education focuses on proximate causes of disease ? infection, trauma, cellular regulation, etc. ? as opposed to evolutionary understandings of how our traits and responses came to be in the first place. What evolutionary insights are there for clinical medicine?
Medical conditions can occur when there is a mismatch between our evolved bodies and our particular society and environment. This idea challenges some of our notions of disease.
Hardly a day goes by when I don?t see patients with lactose intolerance, allergies, obesity, anxiety, near-sightedness, ADHD, and flu symptoms. The lactase gene spread rapidly in historical populations with dairy husbandry. But 70% of the world?s population is lactose intolerant, all of whom are ?normal? in the context of their environments that were, until recently, lactose-free.
Allergies and autoimmune conditions are more common in developed societies where infections occur less frequently. This suggests a mismatch between our evolved immune system and our current environment. Recent evidence suggests that the incidence of autoimmune Crohn?s disease has risen in places where the incidence of gastrointestinal worm infection has fallen.
Obesity likely represents a mismatch between our food preferences which evolved in environments of relative food scarcity, and modern environments with increased food availability and decreased activity levels.
Anxiety may have been an evolutionarily useful response ? think of the advantage of being jumpy when you hear rustling in the tall grass in an African savannah ? but now may be an inappropriate expression for our current environment.
According to a recent study, children who are genetically susceptible to near-sightedness are less likely to become nearsighted if they spend more time outdoors.? This suggests near-sightedness may in part result from a mismatch between the outdoor environment in which we evolved and modern indoor activities such as reading and playing video games.
With an evolutionary perspective, conditions such as attention deficit hyperactivity disorder may be conveyed not as a disease, but rather a mismatch between a patient?s evolved nature and our particular society?s educational expectations. In all these conditions, an evolutionary approach helps clinicians and patients see medical conditions as contextual, rather than as an inherent defect. Evolution ? natural history ? becomes relevant.
Fever, cough, vomiting, diarrhea, etc. are evolutionary host defenses to expel infections, not, as patients often believe, infections themselves. Nonetheless the suffering can be marked. If treatment is provided to alleviate these symptoms, will our body?s defense against infections be weakened? This area is ripe for additional scientific research.
Like the early days of pharmacology and microbiology, it is too soon to predict the extent of clinical relevance that an explicit evolutionary understanding can yield. Evolutionary thinking has already directly impacted clinical medicine in areas such as genetics and vaccine design. Evolutionary principles also inform public health measures, such as the campaign to avoid inappropriate antibiotic use in humans and livestock to help prevent the evolution of resistant pathogen strains.
Just because a trait evolved does not make it good or bad. Evolution itself is impersonal and morally neutral.? It is up to us to provide deliberate values into the blind shuffle of evolutionary selection. An evolutionary scientific understanding provides greater wisdom into health and illness. Even in this world of technological marvels, the ?history and physical? (H&P) is often emphasized as a physician?s most valued diagnostic strategy. In essence, evolution is history. With an evolutionary perspective, the ?H? in ?H&P? can be understood and appreciated at a deeper level.

DENVER ? Colorado?s two Democratic senators are pushing back at a Denver Post story Thursday that reported they are waffling on supporting legislation to ban assault weapons due to political concerns.
In December, roughly a week after the Newtown shooting that killed 20 first-graders, Sen. Mark Udall and Sen. Michael Bennettold?FOX31 Denver that they wanted to see a new ban on assault weapons.
?I?ve come to the conclusion that military style weapons really don?t have any place in our society,? Udall said. ?We ought to reinstate the assault weapons ban that served us well for 10 years from 1994 to 2004.?
Bennet followed suit the same day with a statement of his own calling for a comprehensive approach.
?The shooters in Aurora and Newtown should not have had access to the guns or magazines they used,? Bennet?s statement read. ?I believe a combination of improved access to mental health services, restrictions on certain weapons intended for the battlefield, and elimination of the gun show loophole are sensible steps that can reduce our children?s risk.?
On Thursday, a few hours after Sen. Dianne Feinstein, D-CA, rolled out her revamped legislation to ban assault weapons, the Denver Post?s Allison Sherry tried to find out if Udall and Bennet were on board.
Both senators?addressed?the need to find a balance that also works for sportsmen and stopped well short of saying they will support the specific legislation, which would permanently outlaw more weapons than the 1994 ban, which, unlike the new proposal, had a sunset provision that took affect 10 years later.
Sherry termed the apparent change in position as ?confounding?.
But both staffers for both Udall and Bennet insisted Friday that they are simply looking over the specific legislation introduced Thursday, and that, while they are indeed ?uncommitted? on Feinstein?s bill, their position in favor of a general ban on assault weapons has not changed.
?First and foremost, Sen. Bennet?s concern is that we find solutions that will be effective,? said the senator?s spokesman, Adam Bozzi, on Friday. ?He has already called for an assault weapons ban. He?s going to take a serious look at this specific bill, which was introduced yesterday ? as he will with other bills when they are proposed ? before he decides which he?ll support.
?His concern is that we find solutions that will be effective and he?ll look into data that demonstrates how this will help limit or stop these events.?
Udall?s office also responded to FOX31?s request for a clarification.
?Sen. Udall has clearly said that he believes an assault weapons ban should be considered as part of a comprehensive solution to reduce gun violence,? said Mike Saccone, the senator?s spokesman. ?But details matter. He is thoughtfully reviewing the legislation introduced yesterday, which is different from the 1994 ban, to determine if it conforms to Colorado and the West?s heritage of safe, responsible gun ownership.?
Bennet and Udall have not been among the more outspoken lawmakers pushing for gun control following the Newtown shooting.
Colorado Rep. Ed Perlmutter, D-Golden, is co-sponsoring the House version of Feinstein?s assault weapons ban. Thursday, at the elaborate press conference to introduce the legislation, Perlmutter read a letter from the family members of seven victims of the Aurora theater shooting who want lawmakers to ban assault weapons.
Also, Rep. Diana DeGette, D-Denver, is sponsoring a ban on high-capacity magazines in the House.
But unlike those two lawmakers from safe Democratic districts, Udall and Bennet represent all of Colorado, a western state with a long tradition of gun ownership but also a state that is shifting left politically.
A survey?released?Monday from Project New America and pollster Chris Keating found that 61 percent of Coloradans support a ban on assault weapons, and that other gun control proposals have even higher support in the state.

Amazon's Cloud Drive Photos for Android gets auto-save feature, additional functionality
Despite being a little too busy scooping up companies and striking new streaming deals, Amazon hasn't exactly been overlooking its set of mobile and desktop applications. Most recently, the online retail behemoth announced a refresh was on hand for Cloud Drive Photos on Android, providing users of said app with an auto-save option that, as you likely guessed, allows pics to be automatically uploaded to ones cloud account -- this, according to Amazon, was "the most requested feature from our customers." Better yet, version 1.2 also brings camera integration to devices running Android 4.0.4 or later, which will make it easier for some folks to quickly snap a shot and save it to the cloud-based drive without too much effort. Other tidbits like multiple file downloads and uploads made it to this release as well, making for a pretty plentiful update overall -- links to download from Google Play or Amazon's own app store can be found down below.
